Membership Hold or Cancellation Form

Warning:  This form is for either placing your membership on hold for 1 month or complete cancellation of your membership.
If you are trying to cancel 1 class time, login to your portal and click the cancel button for the 1 class you will not be able to attend.

Membership Hold Policy:

A minimum of 5 days notice before the month you will not be taking lessons is required.

This holds your place on our calendar for when you return in a month!

Only 1 month out of a calendar year is permitted for hold privileges.
The month you are gone the membership fee is paid but the month you return the membership will not be charged.

Membership Cancellation Policy:

A full calendar month notice before the last month of lessons is required to waive the cancellation fee.

Example, if you wish July to be your last month of lessons, then you must let us know by submitting the form below by the end of June.

If appropriate notice is not given, a $50 late fee will be charged upon cancellation.
